What Is Tonsillectomy
The tonsils are two pads of lymphoid tissue at the back of the throat. They form part of the immune system — helping to identify germs in early childhood. In some adults, however, the tonsils become a repeated source of infection, obstruction or ongoing discomfort rather than protection. When this happens, removing them is the most effective solution.
Adult tonsillectomy completely removes both tonsils — including the capsule — by dissecting the space between the tonsil and the surrounding muscle wall. No tonsil tissue is left behind. The procedure is performed through the mouth with no external incisions and no visible scars.
Unlike in children — where obstructive symptoms are the most common reason for surgery — the most common indication for tonsillectomy in adults is recurrent or chronic tonsillitis. Antibiotic resistance and more developed tonsil tissue make medical management less effective in adults over time.
How It Works
Tonsillectomy involves Five precise steps:
- General anaesthesia — You are completely asleep throughout. No local anaesthesia-only approach is used for tonsillectomy.
- Mouth access — A small retractor holds the mouth open, exposing both tonsils clearly. No external cuts are made.
- Tonsil dissection — The surgeon carefully dissects the tonsil away from the surrounding muscle, removing it completely including its capsule. Techniques include cold steel dissection, diathermy, harmonic scalpel or coblation — selected based on the patient’s specific anatomy and surgeon preference.
- Haemostasis — All bleeding points are carefully controlled using diathermy or sutures. Thorough haemostasis is critical — adults have a higher bleeding risk than children.
- Closure — No stitches are needed. The wound heals naturally from within. Both sides are completed before the procedure is finished.
The procedure takes 30-45 minutes and is performed as a day procedure in most cases.

What to Expect After Adult Tonsillectomy
Timeline:
Days 1-3
Pain is significant — particularly on swallowing. This is normal and expected. Take prescribed pain relief regularly — do not wait for pain to peak before medicating. Drink cool fluids constantly. Eat soft foods. Rest completely. A white/yellow coating will appear at the back of the throat — this is normal healing tissue, not infection.
Days 4-7
Pain typically peaks around days 4-5 as healing scabs form in the throat. Continue all pain medication as prescribed. Hydration is critical — dehydration is the most common reason adults return to hospital after tonsillectomy. Keep drinking even when it hurts.
Week 2
Pain gradually improves after day 7-8. Most adults feel significantly better in the second week. Many return to desk-based work after 10-14 days. The white coating gradually disappears as healing progresses.
Full recovery
Full recovery commonly takes around 10-14 days. Some adults take up to 3 weeks for complete comfort. Pain on swallowing is the last symptom to fully resolve.

❌ Do not take aspirin — it significantly increases bleeding risk
❌ Do not allow hard, sharp or scratchy foods — crisps, toast, crackers, nuts
❌ Do not do strenuous exercise or heavy lifting for two weeks
❌ Do not return to work until pain is manageable and you are off strong opioid pain relief
❌ Do not ignore bright red bleeding from the mouth — seek emergency care immediately
❌ Do not smoke — smoking severely delays healing and increases complication risk

Adult tonsillectomy carries a higher complication rate than paediatric tonsillectomy. Adults have more vascular tonsil tissue, longer recovery and a higher bleeding risk. Our team takes every precaution to minimise risk.

Post-Operative Bleeding
The most serious complication. Bleeding is more common in adults than children. It can occur at any time from the day of surgery up to 21 days afterward. Post-tonsillectomy bleeding that does not stop requires immediate emergency care and potentially return to theatre. Risk is approximately 3-5% in adults.

Dehydration
Pain on swallowing makes drinking difficult. Adults who do not maintain adequate fluid intake during recovery can become significantly dehydrated — requiring IV fluids and hospital readmission.

Pain
Adult tonsillectomy pain is more significant and lasts longer than in children. Pain peaks around days 4-5. It is well managed with prescribed pain relief taken regularly and on schedule — but it requires consistent medication management throughout recovery.
