How It Works
RLE involves Five precise steps:
- Anaesthesia — Anaesthetic eye drops numb the eye completely. RLE does not require general anaesthesia in most cases. You remain awake and comfortable throughout.
- Micro-incision — The surgeon creates a tiny self-sealing incision of approximately 2-3mm in the cornea. No stitches are required and the wound heals naturally.
- Lens emulsification — A specialised ultrasonic probe is inserted through the micro-incision. High-frequency sound waves break up the natural crystalline lens into microscopic fragments which are then gently suctioned out of the eye.
- IOL selection and implantation — A premium foldable intraocular lens, precisely selected from biometric measurements of your eye to match your target refraction, is inserted through the same micro-incision. It unfolds inside the eye and is positioned in the capsular bag where the natural lens sat.
- Self-sealing closure — The micro-incision seals itself naturally without sutures. The whole procedure typically takes 15-20 minutes per eye.
Both eyes are usually treated separately, one to two weeks apart, to allow the first eye to heal and confirm the refractive result before operating on the second.

What to Expect After Refractive Lens Exchange
Timeline:
Day 1 — Immediately after surgery
You are discharged the same day as surgery with a protective eye shield, prescribed antibiotic and anti-inflammatory eye drops and a clear aftercare plan. Many patients notice improved vision almost immediately after the procedure, though some blurring, fluctuation and light sensitivity is normal in the first day or two as the eye settles. Vision in the treated eye will be blurred by the protective shield while it is in place.
Days 1-3 — First few days
Vision continues to improve rapidly. You may experience mild grittiness, watering or light sensitivity — these are normal and resolve within a few days. Use your prescribed eye drops exactly as directed. Avoid rubbing your eye. Rest at home and avoid strenuous activities. Some patients experience temporary halos or glare around lights, particularly with multifocal IOLs, during this early period.
Week 1 — First week
Attend your first follow-up appointment. Most patients can resume light daily activities including reading, watching television and working on a computer. Many patients are able to drive within a week of surgery once their surgeon confirms the vision is adequate. Avoid swimming and dusty or smoky environments.
Weeks 2-6 — Full recovery
Vision continues to stabilise and sharpen as the eye heals and the brain adapts to the new IOL, particularly with multifocal and EDOF lenses where neuroadaptation to the new optical system takes time. Some patients with multifocal IOLs may notice halos or glare around lights, particularly at night, during this period — this is normal and typically reduces significantly over weeks to months as the brain adjusts. Full recovery and the final refractive outcome are typically established within 4-6 weeks of surgery.
Recovery Do's and Dont's:

Retinal Detachment
Removing the natural lens and the vitreous changes that follow can increase the risk of retinal detachment, particularly in highly myopic eyes with longer axial lengths. The role of RLE in myopia is less clear, particularly in younger presbyopic high myopes, where RLE may improve unaided visual acuity but may introduce vitreoretinal pathology, sometimes years later.

Posterior Capsule Opacification (PCO)
The most common complication after any lens surgery. A clouding can develop behind the artificial IOL months to years after surgery, causing blurred vision to return. This is not a new cataract — it is easily and effectively treated with a quick, painless YAG laser procedure in a single outpatient visit and does not represent a permanent problem.

Multifocal IOL Side Effects
Patients who choose multifocal or EDOF IOLs may experience halos around lights, glare or reduced contrast sensitivity, particularly at night. These effects are most noticeable in the early weeks after surgery and typically diminish significantly as the brain adapts to the new optical system.
